From what I've been reading up on the donut is on the inside and it causes a rise in pressure.

Easiest way to check is take a fired case and drop a bullet in. If the bullet stops you've got a doughnut. The only time I know of that it could cause a rise in pressure is if you seat the bullet past the bottom of the neck (into the doughnut). The increase in tension holding the bullet could raise pressures.

Yeah, but if you upsize a .243 to .260 and get a doughnut, you won't be able to fire it to be once fired. The press will probably push it past that doughnut when loading.

Doughnuts only show up after you fire them - the expander will push out the neck unless you don't size the whole length. You can do that to force the case back against the bolt/frame face, but that means putting together some pretty tame fire forming rounds.
